We look forward to partnering with your community for our upcoming vaccine clinic! To secure your vaccine inventory and help us smoothly coordinate with neighboring facilities, please follow these simple steps once your clinic date is announced:

• Specify Vaccines: Ensure each consent form clearly details which vaccines are being requested.

• Verify Signatures: Forms must be authorized by the resident, their POA, or a caregiver documenting verbal POA consent (including the POA's name, relationship, date, and staff signature).

• Include Insurance: Provide a copy of the front and back of each resident's insurance card, or write their complete coverage details on the form.

• Fax Documentation: Send all completed forms and insurance copies to our secure line at (949) 561-5264 (faxso we can verify your supply and lock in your clinic session.
